New Member Handicap Form
 

In order to formally establish an official USGA handicap, you must have a minimum score history of five scores.  You are asked to provide at least five scores (and up to 20 scores) in the form below.  For each score, you will be asked to provide an approximate date and identify the tees played.  If the course is not located in the Greater Philadelphia area, you will also need to provide the city and state of the course.

Please note that rounds should have been completed during the active golf season (April through October in the Greater Philadelphia area).
